"Pop Up" Pfizer Vaccination Clinic Scheduled at HCS

Good Morning,

I hope you are all having a good week thus far!

Throughout the region, school superintendents have been working in cooperation with the Ontario County Department of Health to navigate the ever changing landscape of the COVID-19 global pandemic. As a result of this partnership, HCS will host a completely optional "pop up" vaccination clinic onsite in June.

Pfizer Vaccination Clinic at HCS, Sponsored by OCDOH
Logistics:
  • Dates: June 1 (first in series of 2) and June 22 (second in series of 2)
  • Time: 12:00 - 4:00
  • Location: The clinic will be set up in the foyer outside the HCS District Office.
  • Who May Participate:  This clinic is OPTIONAL and open to any/all individuals ages 12 and up.
  • Registration: To ensure an adequate number of vaccinations are on hand, parents of students and/or any other individuals interested in obtaining the vaccination are requested to complete the interest survey pushed out via Parent Square.  Walk ins will be welcomed based on availability of vaccination.
  • Specific to Students: 
    • Parents may opt to be present with their child(ren) ages 12+ for vaccination OR may complete, sign and return the TWO attached documents with child(ren) ages 12+:
      • COVID-19 Immunization Screening and Consent Form (attached below)
      • Pfizer-BioNTech COVID-19 Vaccine Consent Form for Individuals Under 18 years of Age (attached below)
    • Marybeth Tonkery will support coordination of vaccinations for students ages 12+.
  • Other:
    • OCDOH staff will administer the Pfizer vaccinations.
    • Marybeth Tonkery will be onsite and present throughout the clinic to monitor.
    • All individuals obtaining the vaccination will be required to be monitored in the HCS auditorium for 15 minutes following.
      • Juice and light snacks will be available.
    • All individuals obtaining the vaccination must complete, sign and return the attached COVID-19 Immunization Screening and Consent Form (attached below).
    • All individuals obtaining the vaccination must be available on June 1 and June 22 to receive both (in series of two) vaccinations.
    • All individuals obtaining the vaccination will be required to adhere to COVID related safety protocols (screening, masking, social distancing).

  • Next Steps:
    • If you have a child who is 12 or older and would like to them vaccinated, please complete the interest survey pushed out via Parent Square so that we may determine necessary volume of vaccination. We ask that you provide this input by this Friday, 5/28.
